Tattooing A Minor (Under 18)

•We tattoo minors aged 16 and 17

•No exceptions. No one under 16 (not even by a day)

•Per our local health department we are required to collect an original sealed/stamped notary form signed by the parent. You can find the link to this form below. (No photocopies)

•THE SAME Parent or legal guardian that gets the form notarized must be present during the ENTIRE tattoo process. NO EXCEPTIONS

•At the time of the minors appointment, the parent must provide a valid state or government issued ID, and a copy of the child’s long form birth certificate, along with the required notary form.

•When submitting a tattoo request for a minor, the parent must be the point of contact, and the parent must submit the request online in their own name with all their information filled out. (The minor legally can not sign for themselves, so filling it out in the minors name will delay the process)

•When filling out a request form for a minor, please note that the tattoo is for a minor in the details of the request.

•If we are not provided the original stamped notary form, a parents ID, and the childs long form birth certificate you will have to reschedule and will forfeit your deposit. Please come prepared. We don’t want to have to turn anyone away.
